Test results and treatments at Bellin Hospital, Bellin Family Medical Centers, FastCare clinics and worksite primary care visits are recorded in a single electronic medical record. Physician Partners Ltd. specialists are also able to view these records and their practices are linked for scheduling and billing purposes.



The model begins with Health Knowledge, where claims data from your health benefits consultant and Health Risk Assessment results are gathered. This data allows us to assess health and productivity needs and establish baselines. After implementing a program, the model returns to Health Knowledge where performance ROI is measured.
