Complaint Procedure
Any person who has compliments, patient safety issues or concerns about the care, treatment or services he/she or a loved one received at any Bellin Health facility, that has not been addressed by Bellin Health, is encouraged to contact Bellin's management at 433-7869 or by utilizing our online contact form.
If the concerns cannot be resolved by Bellin, the person has the right to contact any of the following organizations:
