Please Note:
1. Policies labeled as NCDs (National Coverage Determinations) apply to all testing, regardless of whether the test is submitted to WPSMedicare Part B or UGS, for payment. All NCDs, indexed above, link directlyto the CMS (Centers for Medicare and Medicaid Services) Website.
2. Policies labeled as WPS (Wisconsin Physician Service) apply to only those tests submitted to WPS Medicare Part B, for payment. (This wouldinclude those tests performed on-site within the Bellin Medical Group Clinics). The WPS LMRPs, indexed above, are individually linked to the WPS Website.
3. Policies labeled as UGS (United Government Services) apply to only those tests submitted to UGS, for payment. (This would include those testsreferred to the Bellin Outreach Laboratory). The UGS LMRPS, indexed above,are individually linked to the UGS Website.

6. As of 11-20-08, UGS does not have a published LMRP for Pap Smears.At this time, it is suggested to follow the WPS LMRP.
