Again this year we are excited to offer a scholarship award to one of our exemplary individuals. This award is open to senior student athletes who have maintained a GPA of 3.5 or above, who show a high degree of commitment to their academics, athletics, and community. He or she must excel on and off the playing field. Please fill out the scholarship application form and submit with an official transcript, a nomination form, and a letter of reference or nomination to:

The monetary value of the scholarship is $500.00.
To be eligible for the Bellin Health Sports Medicine Night of Champions Scholarship, applicants must meet all of the following requirements:
- Must be a senior in high school
- Maintain at least a 3.5 Grade Point Average
- Demonstrate a recognized contribution to your community, athletic achievement, and academics
- Submit a complete application by March 14, 2008 along with a nomination form and a letter of recommendation or nomination. Click here to download a scholarship application form.
The Night of Champions Scholarship recipient will be selected within the sole discretion of Bellin Health. The scholarship will be made payable jointly to the scholarship recipient and his/her parent with the understanding that the funds must be used to pay expenses related to higher education.
