Preparing for Your First Visit
Welcome to our clinic! We look forward to partnering with you to help you feel your best through your Midlife years. Each initial appointment is with Randi Mann WHNP-BC (formerly Randi Burnham), our credentialed NAMS Menapause Parctitioner.
In order to make the best use of your appointment time, please have the following information completed prior to your appointment and bring it with you!
- Women’s Midlife Health Program Initial Appointment Form (Please download this form, print and fill it out completely and bring it with you to your first appointment).
- If you are under the care of a Bellin Medical Group provider and have all of your diagnostic testing done at Bellin Clinics or Bellin Hospital, then we have electronic access to your medical records. If not, please bring with you or have sent to us any medical records of care you received from providers outside of Bellin in the past 1 – 2 years including your most recent yearly physical or gynecological exam notes, and results of your pap smear, mammogram, thyroid test, cholesterol test, bone density test and colorectal cancer screening test if applicable.
- Bring your insurance card and any applicable co-pay or payment for non-covered services.
- Please bring any medications and supplements that you are currently taking in their original bottle(s).
- Please be sure to arrive 15 minutes early. Plan on your visit lasting approximately two (2)hours.
Also please check with your insurance company prior to your visit for any pre-certification or referrals that may be needed.
