Sports Physicals for Student Athletes
Bellin Health Sports Medicine and Bellin Health Family Medical Centers are offering pre-participation exams for students taking part in the upcoming 2008-09 athletics season.
The exams are $20 per student athlete, with 100 percent of the proceeds being donated back to each participating athlete’s school. No insurance will be filed for these visits. Student-athletes must be accompanied by their parents or have a signed parental consent form at the time of their visit.
Students also are encouraged to have their WIAA Green Forms filled out prior to their physicals. The forms can be downloaded from the WIAA web site, www.wiaawi.org. For a complete list of exam dates and locations, click here.
