Responsible and accountable for providing quality professional nursing care to patients and their families and significant others adhering to the WI Nurse Practice Act, the Rules and Regulations of the WI Board of Nursing, the Nursing Scope and Standards of Practice, the Code of Ethics for Nurses and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, staff, and visitors and working collaboratively with the multidisciplinary healthcare team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged. Provide nursing care in patient homes for a multitude of different medical diagnoses. Perform skills such as IV, wound care, conduct physical assessment, and provide patient education.
Wisconsin RN license required. BSN preferred. If ADN, enrolled in BSN program required. Valid WI driver's license and auto insurance are also required. Must have CPR certification, strong critical thinking skills, and ability to multi-task. Being a good team player, and able to work independently are required.
One year of recent hospital or home health experience is required. IV skills and wound vac skills preferred.
Call rotation: on call one night per week from 4:30pm - 8:00am. Triage patient questions and concerns during this time. May need to conduct visit if appropriate while on call, depends on the patient needs.
Weekend call: every fourth weekend, works full weekend with call. Call on this weekend rotation is from Friday (4:30pm) to Monday (8:00am). Visits during full call weekend are generally Saturday and Sunday from 8:00am - 4:30pm.
Holidays: two per year.
