Don’t Exchange Bellin Quality for Something Less.

If you’re a Bellin patient and you’re searching for insurance on the government’s health insurance exchange, there are many things you need to know. Above all, we want you to choose the plan that is right for you, one that fits your health needs and also fits your pocketbook. We want you to have access to quality care, and we want you to be able to pick a plan that includes Bellin Health physicians. But for many, the process of picking the right Health Exchange Plan, one that meets all their criteria, can be confusing, frustrating and overwhelming. Here are some tips that might help.

Parking - Bellin Health Care Systems


Bellin Health Hospital Center

We offer two options for parking. You may either park yourself or enjoy the ease of our valet service. Available 7 days a week, our valet service helps patients and visitors avoid the hassle of trying to find a parking spot and the traffic congestion around the hospital. Both options are free.

When you arrive at Bellin's main entrance an attendant will greet you, give you a claim receipt and park your car. When you are ready to leave, hand your claim receipt to a valet attendant and your car will be brought back to you.

If you decide to park your own car, please follow the signs or stop at the main entrance where the valet attendant will direct you to the nearest parking lot.

Bellin Psychiatric Center

No valet service is available at Bellin Psychiatric Center. Free parking is located in the lot in front of the building.

Green Bay Health And Athletic Performance Center

Parking is available in the lots adjacent to the building. Please feel free to use the covered areas next to the building to drop off any riders. If you have any questions about parking please call 920 433-7917.

Altrusa House - Bellin Health Care Systems

The 12-bed Altrusa House offers a "home away from home" to patients and families who need to stay overnight during a medical emergency. The house provides a safe, caring and interactive environment to guests referred to the home by the local medical community. Families can enjoy television, launder their clothes, cook their own meals and just relax. The facility is within walking distance, located on E. Mason Street just two blocks east of Bellin Health Hospital.

Guests of the Altrusa House generally live greater than 50 miles from Green Bay, are in financial need or require an extended stay in Green Bay due to medical treatment. If a patient is not quite 50 miles from Green Bay but does not have transportation or the strength to travel due to a medical condition, that person (and family) does qualify to stay at the Altrusa House as long as he/she is receiving medical assistance at Bellin Health Hospital.

The first floor living room and kitchen areas are roomy
and give the Altrusa House that "home away from home" atmosphere.

Each private room is decorated with a different theme and has its own bathroom. The room pictured left is handicapped accessible with two single beds. There are a variety of rooms.

The Altrusa House is operated by the Altrusa Club, a group of professional and charitable women with one common bond: they enjoy helping people. For 10 years, families faced with the stress of medical emergencies have stayed at the Altrusa House. Construction of the new house, completed in September of 2001, was financed through the club's fund-raising efforts.

Is The Altrusa House The Same As The Ronald McDonald House?

It is very similar with the exception that the Ronald McDonald House is only for children patients and their families whereas the Altrusa House is for all ages of patients and their families.

How Are We Referred To The Altrusa House?

The hospital case manager or a member of the nursing staff needs to refer you. They will give you a form to sign, which you bring to the Altrusa House when you register. A Bellin staff member should first call and notify the Altrusa House staff of your need.

How Many Rooms Can We Have When We Stay?

Each family may have one room with two guests. If one guest is a patient having medical treatment at an area hospital that individual may have one supportive person with him/her in the room. Two supportive family members may stay in a room when a family member is hospitalized. A second room may be allowed for two additional family members on a day to day basis, providing there is availability of rooms. If the room is needed for incoming guests from another family, a prompt departure may be required. Rooms should always be left in good order. A second room will be assigned on an individual basis with priority given to new guests as necessary. The Altrusa House must accommodate all hospitals.

How Long Can We Stay At The Altrusa House?

As long as needed for the medical problem.

Do We Have To Leave If The Family Member Is Discharged From The Hospital And Is Not Strong Enough To Travel Home?

No, you and the patient may stay as long as you need to.

If One Of The Guests Is A Child Patient, Do You Have A Crib Available?

No, the Altrusa House does not have cribs at this time. Parents are asked to bring their own port-a-crib if they are not comfortable using a regular sized bed for the child. All children need to be supervised by their parents and never left unattended anywhere in the house.

Are Pets Allowed In The House?

No. There are no accommodations for pets.

Sit and relax in the comfortable seating area of The Backstreet Espresso Coffee Shop. Located in the Laird Welcome Center, across from the Bellin Gift Shoppe and Bellin Retail Pharmacy, Backstreet Espresso Coffee Shop offers a variety of hot and cold beverages and snacks.

Backstreet Espresso is a service of the Bellin Auxiliary and is staffed by volunteers. Because of this our hours vary. All profits from the coffee shop are returned to Bellin Health in the form of equipment purchases, staff scholarships and funding special projects.

The Tree Top Café

Get a bite to eat and take a break at The Tree Top Café. Located on the third floor of Bellin Hospital, it features full meals, homemade soup, a salad bar, deli area, desserts and a variety of snacks. A vending area outside the cafeteria and a microwave oven are accessible 24 hours a day.

Bellin Hospital Gift Shop is located within the Laird Welcome Center lobby area. Our shop is supported by staff consisting of over 25 dedicated volunteers who assist the store in many ways.

Closed on Holidays

Our shop is filled with many types of giftware items, including lines such as Willow Tree, Woodwick Candles, Ethel and Myrtle Jewelry, and Beerntsen’s Candies. We also have many different styles and sizes of mylar balloons, and feature fresh cut flowers and green plant assortments from Schroeder’s Flowers.

If you have questions about your health, looking for ways to live a healthier lifestyle or are simply curious about medicine, Bellin is there for you.

The Family & Community Resource Center offers a wealth on up-to-date information from the Internet, health magazines, books and audio and visual materials. Located next to Backstreet Espresso in the Laird Welcome Center, the center is open daily to patients, their families and the community. The center staff may also be reached by calling (920) 433-3682.

This center is also a United Way 2-1-1 Plus Center, offering people without phone and/or Internet access an opportunity to learn about and utilize the 2-1-1 service. 2-1-1 is a non-emergency telephone number that connects people with services and volunteer opportunities within their community.

How can I obtain a copy of my medical record?
You have the right to view and/or request copies of your health care record. To view your health care record, you must make an appointment with the Release of Information Department at the Bellin site your records are stored. (Photo identification is required.)

To obtain copies of your healthcare record, please download and complete the “Authorization for the Disclosure of Protected Health Information” and send directly to: Release of Information at the Bellin Site your records are kept that you are requesting. 

Authorizations may also be faxed to (920) 433-7910.

Can I obtain medical records on someone other than myself?
If you are requesting records on someone other than yourself, you will need to provide legal documentation verifying legal guardianship, power of attorney, personal representative, etc. Biological parents may request copies of their minor child’s records as long as their parental rights have not been terminated.

How long does it take to obtain copies of my medical records?
It takes approximately 3-5 days to complete a request once it has been received. We realize there are circumstances when your medical record will require immediate disclosure. If this is your situation, please call Release of Information to discuss your needs. 

Is there a cost to obtain a copy of my medical record?
There is no charge for copies of your medical record if it is mailed directly to another physician or healthcare facility. Records requested for any other reason may be subject to a processing fee. Please contact Release of Information for more information.

Who can pick up my medical records?
If someone other than the patient will be picking up copies of medical records, that person must bring written authorization from the patient. Photo identification is also required when picking up medical records.

What if I find an error in my medical record?
If you believe that information within your medical record is incorrect or incomplete, you may request to have that information corrected. If we determine to the best of our knowledge your information is accurate and complete, or if Bellin Health did not create the information, we may deny your request. However, if we deny any part of your request, we will provide you with a written explanation of our reasons for doing so. Requests to make a correction to your records must be in writing and ask that you complete the “Request to Amend Protected Health Information” form.

We will make every attempt to protect the privacy of patient information so that such information is not heard, read or otherwise shared with others for any reason other than to promote the best healthcare outcomes for our patients.

In order to disclose protected health information, we require signed authorization from the patient or legal guardian. The healthcare record is the property of Bellin Health and is maintained for the use of the Hospital, Clinic, Medical Staff and for the benefit of the patient. You may request copies of your health information at anytime, with reasonable notice and payment of copying costs.

How to Obtain Copies of Your Medical Records
In order to obtain copies of your medical records, please download and complete the “Authorization for the Disclosure of Protected Health Information” and send it to the appropriate Bellin site. Authorizations may also be faxed to (920) 433-7910 Bellin Hospital or (920) 433-6779 to Bellin Medical Group.         

Turnaround Time
It takes approximately 3-5 days to complete a request once it has been received. If you need your request sooner, please contact Medical Records.

Copy Fees
There is no charge for copies of your health care records if they are mailed directly to another provider or health care facility. In all other cases, there may be a copying charge to receive copies of medical records. Please contact the Medical Records Department for specific copy fees.

Picking Up Health Care Records
If you plan to pick up copies of your medical records, you must provide us with photo identification for verification purposes. If you are unable to pick up your own records and someone else is picking them up for you, please send a note giving Bellin Health permission to provide your medical records to that person.

Right to Amend Your Protected Health Information
If you feel that your medical records are incorrect or incomplete, you may ask us to amend that information. You have the right to request an amendment for as long as the information is kept by or for Bellin Health. If you are requesting an amendment to your medical record, please download, complete and return this form to:

In certain cases, we may deny your request for an amendment if information: (1) was not created by us, if the person or entity that created the information is no longer available to make the amendment; (2) is not part of the health information kept by or for Bellin Health; (3) is not part of the information which you would be permitted to inspect and copy; or (4) is accurate and complete.

Authorization for Verbal Communication of Health Information
Patients have an opportunity to sign an authorization that will authorize Bellin Health to contact a patient at home and leave detailed messages on their answering machine or with someone else they may have specifically authorized us to share their healthcare information with. In the event we need to reschedule an appointment or change a medication, we would also able to leave detailed information instead of a generic message. Right to Receive an Accounting of Disclosures
You have a right to request an "accounting of disclosures." This is a list of those people with whom Bellin Health may have shared your health information, with the exception of information shared for purposes of treatment, payment or health care operations or when you have provided us with an authorization to do so. We will provide the list at no cost once during each 12-month period. For any additional requests, we may charge you a fee for the cost of providing the list. We will notify you of the fee and you may choose to withdraw or modify your request at that time before any costs are incurred. Your Right to Choose

The staff of Bellin Hospital recognizes your right to understand as much as possible about your health care. It is your right to accept or refuse treatment. Advance Directives help protect these rights. The following gives you information about Advance Directives, and Bellin Hospital's policies and procedures respecting the exercise of such rights. What are Advance Directives?

Advance Directives are instructions written in advance that state your choices about health treatment or name someone to express those choices for you if you are no longer able to do so. A Living Will or Power of Attorney for Health Care allows you to make key decisions about your future health care.

What is a Power of Attorney for Health Care?

It is a written document that names another person to make health care decisions on your behalf should you become incapacitated.

The agent must know you well enough to be able to give the same directions for your health care as you would if you were not incapacitated. The agent should be someone you trust, such as a relative, who is at least 18 years of age and is of sound mind. Your agent cannot be your health care provider or institution. The document must be dated, signed, and witnessed by two people. These witnesses, however, cannot be relatives, potential heirs, or your health care providers.

What does incapacity mean?

Incapacity means you are not able to receive information and decide what treatment to receive, or to make your wishes known about your health care. Two doctors or a doctor and a licensed psychologist must examine the person and determine if he/she is incapacitated.

What is a Living Will?

In Wisconsin, a Living Will is a document in which you can express choices about your care if you should become terminally ill or be in a persistent vegetative state. Most other states also have their own Living Will forms. If you have one from another state, please inform your nurse, hospital chaplain, or resource case manager.

The Living Will must be signed, dated, and witnessed by two people. Witnesses, however, cannot be relatives, potential heirs, or your health care providers.

Is there a difference between the Living Will and the Power of Attorney for Health Care?

Yes, there is a difference. The Living Will is a statement directed to your physician describing the kind of care you wish to receive in the event you are diagnosed with a terminal illness or if you are in a persistent vegetative state. The Power of Attorney for Health Care, on the other hand, assigns broad power for making health care decisions to another specified person in the event that you become incapacitated. So while they are two different decisions to be made, Bellin has them conveniently combined into one Advanced Directive document.

Do I need an Advance Directive?

There is always a chance you may be seriously ill, injured, or otherwise incapacitated and not able to make decisions for yourself. Having an Advance Directive will help you to ensure that your wishes are carried out.

How long is my Advance Directive in effect? May I change my mind?

An Advance Directive is in effect until you change or revoke it. If you do so, however, be sure to tell your health care agent and anyone else to whom you have given copies of the document.

  1. As patient advocates, health care providers need to inform patients of their right to make decisions concerning their medical care, including the right to formulate Advance Directives.
  2. It is in your best interest to have an Advance Directive; if you do not have one, you will still receive care.

Hospital Bill

As part of your "Patient Bill of Rights," we will provide you with an itemized copy of your hospital bill on request. Professional fees of physicians are not included and you will be receiving a separate billing from the physician's office.

All patients are required to present their current insurance card(s) and will be asked to assign insurance benefits to Bellin Hospital. Your insurance may require you to obtain prior approval and to complete necessary claim forms before receiving services.

Your insurance contract is between you and the insurance company. As a courtesy to you, we will be happy to prepare and submit a claim to your insurance carrier. You are responsible for all charges not covered by your insurance plan. If, after a reasonable period of time, your insurance carrier has not honored your claim, you will be responsible for full payment of total charges.

Medicare patients are required to present their Medicare card upon admission to the hospital. Medicare deductible will be payable unless covered by additional insurance and those benefits assigned to the hospital. Personal items and all non-covered services are your responsibility.

Please present a valid Medical Assistance identification card upon admission. If you do not show your card or your card is invalid, the charges could become your personal obligation.

Accidents and Injuries

Medical expenses incurred as a result of a motor vehicle accident or a liability are your personal responsibility. However, as a courtesy to our patients, the hospital will bill your health insurance, government programs, and liability insurance.

Community Care

Bellin Health is proud of our mission to provide quality care to all who need it, regardless of a person’s ability to pay.

The Bellin Health Community Care Program helps people who may need financial assistance to pay for their Bellin Health services. The program provides help to patients who have a financial need regardless of race, creed, color, sex, national origin, sexual orientation, age, or source of income.

Bellin Health has contracted the Resolute Group in the medical hospital to assist those that do not have insurance coverage. the Resolute Group may contact the patient/guarantor while at the hospital or after being discharged. Their goal is to determine if the patient would qualify for any other coverage or public assistance and also assist the patient in applying. 

The Bellin Health Community Care Program can be used for eligible services after all other financial and third-party resources are exhausted. Routine and preventative care, accounts with balances of $100.00 or less are not eligible for Community Care adjustments.

This program will not consider charges from other agencies or providers. You must contact their office to make payment arrangements. We do not pre-qualify anyone for the Community Care program. You must have a balance with Bellin Health over $100.00 to apply.

Patients qualify for the Community Care Program based on their income, assets, and other debts. The amount of financial help Community Care provides may be all or a percentage of the amount a patient owes and will then be considered for the next 3 months. A letter will be sent to the patient/guarantor upon completing our consideration of the application.  Any remaining balance after adjustments are made is expected to be paid following the payment guidelines included in the letter.

If you would like to apply for this assistance program, please contact Bellin’s Business Office at 920-445-7210 at your earliest convenience. You will be sent an application and instructions on how to complete it. If you have difficulty completing the application, assistance will be provided. It is requested that information be received in our office in 10 business days.

The National Health Insurance Industry determines how insurance companies divide payment of your account. All insurance overpayments will be confirmed with the insurance companies involved and will be refunded in accordance with state and federal regulations.

Arranging for Payment

Bellin Hospital's financial advisor will help you find the best payment plan for charges not covered by insurance. Information is available on our Financial Assistance Policy and our Self-Pay Discount.

Payment may be made in cash or by credit card (Visa, Mastercard, or Discover) in the Patient Registration Department at preadmission, admission, or before being discharged. Payment may also be made upon receipt of your bill to the Bellin Cashier Office.

If unable to pay within 30 days, payment arrangements of up to 36 months should be approved and scheduled by calling 920-569-4122 or by going to http://www.helpfinancial.com. Payment option and financial assistance information is available here.

Community care is available to all patients who show financial need; however, before applying to this program the hospital will help you explore all other options for financial help.

To this end, the multi-disciplinary Ethics Committee may be consulted by physicians, other health care personnel, patients, and family members when ethnical issues arise.

Members of the Ethics Committee are available to offer guidance regarding the ethical aspects of your medical situation. Pastoral Care

For your spiritual needs and your family's, members of our Pastoral Care Department are available 24 hours a day. To contact a member of our pastoral care department, ask your nurse for assistance or call 920 433-3482 or 920 433-7934.

Communion is distributed to Catholic patients daily (except Saturday) and upon request. Protestant patients wanting to receive communion should ask their nurse to notify a chaplain. Other sacraments are also available upon request.

We also encourage your own clergy to call on you during your stay.

Resource Case Management

Our case managers are available to work on your behalf. They can meet with you to discuss your personal, emotional, social, financial, or care needs which could impact your successful recovery from your illness or surgery. They are responsible for working with you on insurance requirements and your discharge plan. Some examples of this include arranging for a nursing home placement, home care, transportation, referrals to community agencies or supportive counseling. Your resource case manager can be reached by asking your nurse, or by calling 920 433-3549 and ask for "Case Manager".

Unity Hospice

Unity, a cooperative effort among Green Bay's three not-for-profit hospitals, provides a variety of services through its hospice, palliative, and bereavement care programs to meet the needs of patients with a limited life expectancy and their families. Diabetes Services

A team of registered nurses and dietitians, specially trained in diabetes education, work directly with each patients primary care physician. Our purpose is to allow the person to learn about the disease and how to best manage it while promoting independence in self-care. Patients can access diabetes services through either Green Bay location: Family Medical Center - Green Bay Webster Avenue and Family Medical Center - Ashwaubenon.

Bellin Hospital has a volunteer staff of approximately 400 individuals, from teenagers to senior citizens, assisting in many departments. Their activities include the support of our nursing staff, delivery of mail and flowers, and staffing of the information desk in the lobby. Our Hospital Hosts assist patients in getting around the hospital and moving equipment from department to department.

To Call Your Nurse

Our bedside communication system makes it possible for you to talk with nurses at the nursing station from your bed. You will also find a call button in the bathroom should you need assistance. Your nurse will show you how to use these convenient devices.

Your Room

Your physical surroundings are just as important in the hospital as they are in your own home so we have tried to make them as comfortable as possible. If the type of room you requested is not available when you are admitted, please advise your nurse. We will make the necessary arrangements to switch you when such a room becomes available.

Your Bed

Your bed is operated electrically. Your nurse will show you how to raise and lower it.

The Bedside Cabinet

To avoid belongings being lost, please be sure to store items such as eyeglasses, contact lenses, hearing aids and dentures in the cabinet next to your bed.


Side rails on your child's crib should be kept in an upright position at all times, unless you are standing next to the crib.

Private Rooms

Private rooms may or may not be available. When available, an additional ten dollars per day will be charged for the private room.

Using Your Television

For your enjoyment, we feature a variety of local cable channels. Refer to the cable channels guide at your bedside stand. Television services are provided free of charge.


Menus are distributed on a daily basis. If you need assistance, a dietetic technician will assist you with menu selections and answer any food and nutrition questions you may have.


The Green Bay Press-Gazette, Green Bay News-Chronicle and USA Today are available from coin-operated machines just off the South Elevators near the cafeteria on the third floor. The Green Bay Press-Gazette is also available from a coin-operated machine near the Clinicare entrance, or it can be delivered to your room. Please check with your nurse. The Green Bay News-Chronicle is available free of charge at your nurses station.

Mail and Flower Delivery

You are welcome to receive mail during your hospital stay. It is delivered daily and can be addressed as follows:

Please be aware that patients in our Critical Care, Intermediate Care and Pediatric Intensive Care Units may not receive flowers for health reasons.
