Location - Bellin Health Care Systems

Location

The Bellin Inpatient Rehabilitation Department is located in the 725 Webster Medical Building (across the street from the main hospital) on the second floor. The unit contains 12 inpatient beds, a group dining area and multiple treatment rooms.

Services - Bellin Health Care Systems

Services

Our Inpatient Rehabilitation patients follow a step-by-step process within the Unit.

  • Assessment – Upon admission, each patient receives a comprehensive evaluation
  • Treatment – Individualized treatment plans are created for each patient and are managed by a team of healthcare professionals
  • Education – Patients and their family members are educated on the rehabilitation process, activities and precautions to be taken after the patient has been discharged
  • Follow Up – Patients receive follow-up phone calls after discharge to ensure they are progressing with their recovery and to address any additional needs. Follow-up appointments are scheduled as needed

Inpatient Rehabilitation services include:

  • 24-hour physician availability
  • 24-hour rehabilitation nursing
  • Intensive level of therapy, with a minimum of two of the following therapies, with three sessions daily, five days a week :
    • Physical therapy
    • Occupational therapy
    • Speech therapy
  • Other inpatient care services determined necessary to address patient’s specific medical condition

  • Amputation
  • Orthopedic disability
    • Long bone fracture
    • Joint replacement*
    • (Age 35 or greater; BMI 50 or greater)
  • Cerebral vascular accident (CVA), Stroke
  • Brain injury, traumatic and non-traumatic (Rancho Level 5 or higher)
  • Major multiple trauma
  • Neurological disorder
    • Multiple sclerosis
    • Parkinson’s disease
    • Polyneuropathy

Hospital & Inpatient Services

Bellin provides inpatient rehabilitation services for those individuals needing physical, occupational or speech therapy in conjunction with national rehabilitational care partner, Rehab Care. By offering inpatient rehabilitation, we can provide our patients with a seamless continuum of care. With a better than average length of stay of 10-12 days, we provide superior outcomes.

Our 12-bed, private room inpatient rehabilitation facility offers the intensive rehabilitation services you need to make the transition from a hospital setting to your home or a community based care environment.

The Medical Director, Dr. Jason Schave, is a physician specifically trained in rehabilitation who coordinates the overall medical management and physical treatment program and oversees the multidisciplinary rehabilitation team.  When combined with the rest of the team, he delivers the right mix of medical and rehabilitative care to produce the best outcomes.

The Joint Commission's Certificate of Distinction for Primary Stroke Centers recognizes centers that make exceptional efforts to foster better outcomes for stroke care. Our achievement of certification signifies that the services we provide have the critical elements to achieve long-term success in improving outcomes. It is the best signal to your community that our quality care is effectively managed to meet the unique and specialized needs of stroke patients.

The Joint Commission's Primary Stroke Center Certification program was developed in collaboration with the American Stroke Association. It is based on the Brain Attack Coalition's "Recommendations for the Establishment of Primary Stroke Centers."

Bellin Health’s Inpatient Rehabilitation Unit has also passed the survey given by the Center for Medicare and Medicaid Services (CMS). This survey examines programming and service offerings, as well as adherence to regulatory and patient safety standards.

Access - Bellin Health Care Systems

Access

The Inpatient Rehab team and Medical Director will review each case to determine if inpatient rehabilitation is the best course of treatment.

  • Minimum requirements for patients entering Bellin Health’s Inpatient Rehabilitation Unit include:
  • Medical stability
  • 18 years or older (adult-only facility)
  • Sufficient mental alertness
  • Ability to participate in therapy programs
  • Significant improvement expected within reasonable amount of time
  • Evidence of potential functional gain from intensive therapy

Referral

Bellin Health’s Inpatient Rehabilitation Unit accepts referrals from any hospital, medical practice, nursing home or home health agency. If you have an issue that may require rehabilitation, talk with your primary care physician or specialist to receive a referral to our program. Executive Health Program

The Bellin Health Executive Health Program is designed to provide a total health analysis in a format that meets the needs of people who desire exceptional quality and respect for their time.

Participants in the Executive Health Program will experience a quality medical examination conducted by Dr. Daniel Koster, the medical director. Participants will also be seen by medical specialists whose services are deemed necessary based on family and personal medical history, age, gender and special concerns.

Every executive health participant receives a physical examination and a consultation with specialists in nutrition and physical fitness.
