2017 Black Tie & Blue Jean Extravaganza
The Bellin Health Foundation is now accepting applications to be named the recipient of the proceeds from the 12th annual Black Tie & Blue Jean Extravaganza being held in September 2017.
The Black Tie & Blue Jean Extravaganza is the only Bellin Health Foundation Fundraiser. It is an annual event where the event committee decides on the beneficiary of the funds raised to benefit a Bellin program or service.
The net proceeds of the Black Tie and Blue Jean event this past September were over $111,200, raised for the Child Life Program. Past recipients have included the Bellin Health Telemonitoring Program, BPC Adolescent/Child Unit, Bellin KidCare, The Cancer Team, the Kuehne Imaging Center, Bellin Childhood Obesity Prevention and Treatment Initiative, THRIVE, Bellin Expressive Arts in Medicine (BEAM) Program, the Bellin Patient Assistance Fund and Nursing Education.
We are now accepting applications for your nomination of a Bellin program or service. Please submit your nomination via the attached form and include any additional information to support your case. Please be specific and include a budget.

- If you have submitted an application in the past and have not been selected, you may resubmit an application.
All applicants will be asked to appear for a 5 minute question and answer session in January in front of the Black Tie & Blue Jean Committee in the Kresge Center. The recipient of the proceeds will be chosen at the end of that meeting and notified by the end of that day.
The recipient of the funds will be asked to provide volunteers to participate on the 2017 Black Tie & Blue Jean Committee, either on the Live Auction, Silent Auction or the Sponsorship Committees.
All applications must be received by December 31st, 2016.
