Event Description:

This free advance care planning workshop led by trained facilitators helps participants understand and complete a Power of Attorney for Health Care document.  A Power of Attorney for Health Care document is a written statement about how you want your health care decisions made when you can’t make them yourself.  We will provide answers to your questions about this important legal document. 

You are encouraged to bring the individual(s) you would choose as your health care agent(s).  All forms will be provided.
