Advance Care Planning is a process that helps you reflect on health care choices you may have to make in the future, select a person you trust who can make choices for you if you cannot make them for yourself, and make a written plan for the future.
Everybody age 18 and older should have an Advance Directive. Family members do not have automatic legal authority to make health care decisions, if you are not able to.
An Advance Directive (Power of Attorney for Health Care) is a document where you can write down your goals values and preferences. It allows you to:

Formally name your person who can make choices for you, only if you are not able to make choices for yourself.

Advance Care Planning Tools on MyBellinHealth
MyBellinHealth now offers you the ability to:
- Check to see that we have your Advance Directive documents on file
- Upload any missing documents such as your Advance Directive, Living will, a Power of Attorney, or a Physician Order for Life Sustaining Treatment (POLST) signed by your physician.
To access the Advance Care Planning page on MyBellinHealth:
- Login to your MyBellinHealth account.
- From the Health menu at the top of the page, select Advance Care Planning.
The Advance Care Planning page will list any documents you already have on file, and allow you to upload new one. After you upload a new file, our staff will review it. We will send you a MyBellinHealth message once your file has been accepted, or if there are issues with the document.
