Advance Care Planning at Bellin Health

Advance Care Planning is one of the best gifts you can give to yourself and to your loved ones.  We can help.

What if you had a sudden illness or injury and could not speak for yourself?
Who would you want to speak for you?
What would you want them to know about your health care wishes?

Advance Care Planning is a process that helps you reflect on health care choices you may have to make in the future, select a person you trust who can make choices for you if you cannot make them for yourself, and make a written plan for the future.

Everybody age 18 and older should have an Advance Directive.  Family members do not have automatic legal authority to make health care decisions, if you are not able to.

An Advance Directive (Power of Attorney for Health Care) is a document where you can write down your goals values and preferences.  It allows you to:

  1. Formally name your person who can make choices for you, only if you are not able to make choices for yourself.

  2. Express your wishes related to treatment options.

  3. Request any special ceremonies or traditions.
